This one comes from a company I visited 20+ years ago. Field returns were extremely high, and the stated MTBF for the 10 kW power supply design was 200 hours. This was mentioned in a meeting, so I asked if they meant 200 thousand hours. No, it was just 200. They made their money on the disposable bits for welding tools, so no one was concerned about the power supply reliability at all. I had some fairly strong suggestions for improving the design, but I didn't get invited back.
